RHC Supported Living Services

What dreams and goals do some of us have growing up?  We often want to live on our own and be independent.  Supported Living/Apartments provides this opportunity to individuals with developmental disabilities who have the same dreams and goals for themselves but may need some assistance to achieve them.

Description Of Residential Services Offered By RHC:

Our Supported Living program consists of individualized services that are tailored to meet the needs of the persons who receive support in the home and community of their choice with staff of their choice.  The services are guided by each person's individualized annual plan which includes the input of the person and all those of his/her team who are involved.  This plan recognizes a person's strengths, preferences and needs.

Support enables individuals to live as they choose and ranges from part-time drop-in contact to 24 hour a day services.  It provides assistance as needed while developing independent living, including areas such as cooking, cleaning, budgeting, and grocery shopping. 

The support continues beyond the living environment to the community and most aspects of a person's life.  It may involve community recreation and memberships, collaborating with a person's work place to assistance with success at work and assistance to visit with family and friends to maintain those relationships.

Funding and Eligibility:

Supported Living services are a possibility for anyone funded through a County Board of MR/DD Supported Living or IO Waiver or by any other agency funding services for individuals needing assistance in some aspect of living.
